Clinical Analysis of Lymph Node Metastasis of Gastric Cancer



Abstract
There were 86 cases of gastric cancer operated with lymph node dissection more than D2 for 4 years from April 1990 to March 1994 at the department of general surgery of Kangnam gen eral hospital public corporation.
Analysis of those cases about lymph node metastasis was done and the followings are the results.
1) Average mean number of total dissected regional lymph nodes was 41.2 per each patient. 59% of all patients had metastatic regional lymph node or nodes.
2) The larger the tumor size, there were the more regional lymph node metastases.
3) The number of metastasized lymph nodes of ulceroinfiltrative and infiltrative type was greater (13.7 & 22.3) than those of ulcerative type (4.5). And N stages greater than N1 were more frequent at ulceroinfiltrative and infiltrative type (45%
and
57%) than those at ulcerative type (31%).
4) The 'poorly differentiated adenomcarcinomas' were more frequently metastasized to the regional lymph nodes (82.9%) than the ' well differentiated adenocarcinomas' (42.8%).
5) The rate of regional lymph node metastasis was related to the depth of the tumor invasion into the gastric wall. The cancers confined to mucosa and muscularis macosae were metastasized to the regional lymph node or nodes in 0%, cancers within
submucosa were in 20.0%, cancers within proper muscle layer were in 46.2%, cancers within subserosa were in 83.3%, cancers emerging from serosal surface were in 85.7% and cancers involving neighboring organs were 100%.
6) The cardial and fundal tumors were more frequently metastasized to regional lymph nodes (90.0%) than the body or antral ones (55.9 and 52.9%).
7) The locations of lymph node metastasis were No 6 node, No 3 node, No 4 & node, No 5 node, No 1 node, No 12 node No 2 node and No 16 node in decreasing orders in all cases. And these were No 6 node, No 3 node, No 8 node, No 5 node, No 4 node
and
No
12 node in antral cancer ; And No 3 node, No 4 & 6 node, No 8 node, No 1 and No 2 & 12 node in body cancer ; And No 3 node, No 4 node, No 6 & 10 node and No 2 node in cardial and fundal cancer.
